COVID-19 PANDEMIC SAFETY PROTOCOL MEASURE

FOOD BADGE APPLICATIONS

The CMOH Office and Public Health Inspectorate, County St Patrick, advised that all Food Badge Applicants and Certificate of Registration Applicants, to place the following documents in a sealed envelope:

1. Receipt of Payment to Penal/Debe Regional Corporation
2. Completed Application Form
3. Medical Certificate completed by the Applicant’s Doctor
4. A copy of ID Card/DP/Passport
5. Two passport photographs

The sealed envelope to be addressed as follows:

            CMOH OFFICE

            Public Health Inspectorate

            County St Patrick.

Deliver the sealed envelope to the Security Officer at the CMOH OFFICE County St Patrick.
